West/Houston/Memorial area.

Please print this form and mail in with full payment or a $150 deposit. The balance is due one week prior to the workshop. Full refunds are given for cancellations 10 days prior to the start date of the workshop. Cancellations less than 10 days will be refunded minus a $75 administrative fee.
